    1st Dose of:

  • 4 months

    2nd Dose of:

    DTap, Hib, IPV, PCV, Rotovirus

  • 6 months

    3rd dose of:

  • 12 Months

    Hep A, MMR, Varicella

  • 18-24 months

    Hep A (2nd Dose), Hep B (2nd Dose), DTaP (4th Dose)

  • 4-6 years

    DTaP (5th Dose), IPV (4th Dose), MMR (2nd Dose)

  • 9 years old

    HPV (1st Dose)

  • 10 years old

    HPV (2nd Dose)

Vaccine Key

DTaP = Diphtheria toxoid, tetanus toxoid, and acellular pertussis vaccine

Hep A = Hepatitis A vaccine
